His decision to design Pearl was influenced by his regular supervision of whale watches at the Ocean Institute, as well as by a cetacean skeleton at the institute. According to him, this manager was redheaded, muscular, and a former army cook these traits were all adapted into Krabs' character. Hillenburg drew inspiration from his former manager at a seafood restaurant while creating Mr. Patrick's personality embodies the nature of the starfish according to Hillenburg, they look "dumb and slow" but are "very active and aggressive" in reality, like Patrick. Many of their characteristics were based on Hillenburg's experiences during his time at the Ocean Institute or inspired by the traits of their species. Krabs, Pearl, and Squidward were the first other characters Hillenburg created for the show.

Symptoms are rabies-like, the infected snail becomes full of rage and foams out of the mouth. It seems that it is further caused when snails are in extreme pain. It is a disease caused by a splinter pricking part of a snail's skin. After Gary goes on a rampage and bites several other Bikini Bottomites, he supposedly started an "outbreak" of MSD and the "infected" began rampaging across the Bikini Bottom.
It is a highly contagious disease which causes the same symptoms but in other sea creatures. Supposedly, after Gary bit Squidward, Grouchy Snailitis appears to mutate even further into Mad Snail Disease. It appears in the episode " Once Bitten" and the books Mistaken Identity and Attack of the Zombies! It is an infection that causes mild symptoms similar to the real-life rabies virus caused by extreme pain. Attack of the Zombies!
